Appointment Request

Use this form to request an appointment with us. While we will do our best to accommodate your requested day and time, please note, your appointment is not fully booked until you get a confirmation from us!

*Cancellation policy: for cancelled or missed appointments within 24 hours of the scheduled appointment time will be charged a fee of $67 and for surgical or dental procedures cancelled or missed within 48 hours a fee of $134 will be charged.  Please let us know if you cannot make your appointment so that we may offer this available time to another pet in need.
